Camp Civitan operates on a first come, first serve basis. ALL application pages must be complete and returned to the office before a camper is confirmed. Medical review must be turned in 4 weeks prior to camp session.
IMPORTANT: Camper will not be fully registered until:
A. Application is signed, completed and received by the Camp Civitan office with the $50 deposit.
B. You receive written confirmation from the Camp Civitan office. Contact us if acceptance/
confirmation letter is not received within 3 weeks.
Please do not assume acceptance.
Written confirmation is the only verification acceptable.
C. You have sent Medical Form and Copy of medical insurance card is attached to the page provided with the medical form.
D. Private pay campers must pay in full prior to attending session.
E. All DDD Authorizations are in the office.
F. Parents/Guardians must contact their Support Coordinator to authorize hours to Civitan Foundation, Inc.
